EDITORIAL

It is with great humility that I have taken up the responsibility of the Editorial-ship of the Journal of the

Pulmonologists of Tamil Nadu. It is a collective efforts of all pulmonologists in Tamil Nadu that we have come to the stage of bringing out the Journal. Having said that it is not an easy job to bring out the issue regularly unless there is a regular flow of articles. I think my job is going to be easy as there are leading pulmonologists, leading and premier medical institutions and hospitals across Tamil Nadu who can contribute their might to make this an Indexed journal and take it forward.

I have decided with my editorial team to bring out articles that would help post graduates, budding pulmonologists and others who are on the look out for a platform to express their opinions. The strength of any journal is the original work. The articles from Tambaram Sanatorium and Apollo Hospitals on Tuberculosis testify to this. No respiratory journal is complete without article on Tuberculosis.

Bronchial asthma is another topic that is making lot of noises in literature. A decade ago every one thought with inhalers, steroids inhalers and regular use of them it is possible to control this disease. But then diseases have their own ingenious ways of circumventing the solutions discovered by us. So there is an urgent need to find out ways and means to find solutions. In this respect the article on Biologicals in asthma highlights how identification of asthma phenotypes and endotypes would revolutionalise the treatment perspectives in asthma. Similarly the article on Bronchial Thermoplasty highlights how one can find solutions for GINA grade 5 asthmatics. Two decades ago bronchoscopy in an asthmatic for any purpose would not be accepted. But the scenario has changed. It is used to diagnose ABPA and also for therapeutic purposes.

Another interesting corner that we have introduced in this journal is View Point where one can express their views frankly. In this issue we have brought out the symbiotic relationship between the interventional pulmonologists and the thoracic surgeons aptly expressed by a thoracic surgeon.

Case reports are the pillars of any journal. It gives confidence to budding pulmonologists the approach and the methods that can be adopted in handling cases. Also will guide us in referring these patients to right people and get them for tele consultation to help them in identifying, diagnosing and treatment of patients. In short, case report presentation is sharing of thoughts and I request readers to critically analyse these as it will be a learning experience for all.

There is one corner named Pictorial CME where X-ray, CT Chest, PFT or bronchoscopy images will be discussed.

Lastly I want to mention that all articles are strictly vetted before publication. But some issues may go unnoticed and that is what I want readers to point out in the forthcoming issue. I intend to have a letters and responses section in the next issue. I request the readers to be brief and with some purpose.

Finally I thank the The Association of Pulmonologists of Tamil Nadu for this great honour to steer forward this Journal JAPT.
